Guidelines to Completing Application for Replacement of Passport

Use Form PP1 (Passport Application Form)

1. A copy of both sides of your valid Omang or a copy of a birth certificate in the case of a minor.

2. Your expired passport.  (If passport is lost attach a police report and declaration of lost passport.   Click here to view and print declaration of lost passport form.)

3. A US$13.00 application fee in the form of a money order, Company Check or Bank Check payable to the Embassy of Botswana.  For Lost Passports, the fee is $152.00.  Do not mail cash. 

4. Two recent passport-size photos with the applicant’s full names printed on the back. 

5. In the case of a married woman who wishes to change the surname include:

♣ a copy of the marriage certificate 

♣ a copy of front and back of applicant’s valid Omang 

6. To complete Section 6, have a Notary Public administer the oath in lieu of the Commissioner of Oaths - they are usually found in banks.  Please do not forget to append your specimen signature in both boxes provided. 

7. Applications must be completed in full to avoid delays.

NOTE

• Old/Previous Passport must be included in the application for replacement/renewal of passport

• Replacement passports cannot be issued without a valid Omang.

• Applicants with expired Omang must go to Botswana for renewal.
