6. Description of the applicant’s duties from the Organization

7. Application letter from the applicant

8. Certified copy of Birth Certificate

9. Certified copy of Passport

10. Certified copy of Marriage certificate (if accompanied by Spouse)

11. Two passport size photos (not computer photos)
