2. Form 3 - Medical Report

3. Letter of Support/Sponsorship Letter

4. Certified copies of Supporter’s particulars, either

· Omang

· Residence Permit

· Permanent Residence

· Naturalization

1. Letter from School stating the duration of the course

2. Certified copy of Birth Certificate

3. Certified copy of Passport

4. Receipt of Payment of school fees (certified at School)

5. Two passport size photos (not computer photos)

6. Certification of Registration (certified) of School by BOTA (Certificate Level Courses) or TEC (Diploma and above)

7. Covering letter from the applicant

8. P500.00

Renewal (The numbers below correspond with the above requirements

a) 1,3,4,5,6,7,8,9,10

b) Original Residence Permit

c) P250.00

APPLICANTS ARE ADVISED TO CONFIRM IF INSTITUTIONS ARE

REGISTERED WITH BOTA AND T.E.C. AND FURTHER CONFIRM WITH T.E.C. AND BOTA IF THE COURSES THEY WANT TO STUDY ARE ACCREDITED.
