1. Form 15 - Application for Residence Permit

2. Form 3 - Medical Report

3. Letter of support from the Guardian/Supporter

4. Supporters particulars, either copy of:

· National Identity

· Residence Permit (Valid)

· Exemption Certificate

· Permanent Residence

1. Certified copy of Marriage Certificate (confirmation by the Headman in case of customary marriage)

2. Death Certificate (if one of the spouses is deceased)

3. Certified copy of Passport

4. Two Passport size photos (not computer photos)

5. P500.00

6. Application letter from the applicant
