1. Form 15 - Application for Residence Permit

2. Form 3 - Medical Report

3. Letter of support from the Guardian/Supporter

4. Supporter’s particulars, either:

· National Identity

· Residence Permit

· Exemption Certificate

· Certificate of Permanent Residence

1. Letter of Consent from the parent(s) if the supporter is not a biological parent

2. Adoption Certificate if the child is adopted (copy of Marriage Certificate if adopting parents are married)

3. Certified Death Certificate or Affidavit if the applicant’s parents are dead

4. Two passport size photos (not computer photos)

5. Certified copies of Passport

6. Letter from School if applicant is a student

7. Application letter from the applicant

8. P500.00
